To receive your $150 rebate, please follow these steps:
1. Purchase one of the qualifying part numbers listed at right
2. You must include original product label which shows UPC BAR CODE label and product part number located on the package or box. Rebate requests submitted without UPC label from original carton may be rejected (example shown below, right).
3. Circle the qualifying purchase on your dated sales receipt, invoice or packing slip.
